For returns related to participation in our Research Study (Protocol: LBL-1001)
Withdrawing from the study:
Your decision whether or not to participate in this study is strictly voluntary. Refusal to participate will not involve penalty or loss. You are free to discontinue participation at any time.
How to notify us that you would like to withdraw:
If you decide to leave this research, contact the research team at (800)747-2045 or email@example.com so that the investigator can:
- Notify the lab to not process any additional samples from you.
- Determine what you would like done with your existing samples.
- Direct the sponsors to refund the cost of any unprocessed tests, less the cost of each study collection kit that has already been provided to you or your practitioner.
What will be refunded to you:
We will refund the amount paid by you to enroll in the study for any sample not yet received or processed by the lab, less the cost of each study collection kit that has already been provided to you or your practitioner.
What we are not able to refund:
Because each collection kit may be different based on the particular aspects of the study protocol, and because we must ensure that all kits are sent to you in a new condition, we are unable to refund the collection kit cost. The cost of each kit will not exceed $50. 3 test participation will have 3 kits.
You will still be responsible for any costs of practitioner consultations, phlebotomy services, and sample shipping you have already incurred, however those charges are not part of the study participation charge and will not have been paid to us by you.
What will happen to your data :
Any depersonalized data previously resulting from the research may still be used as part of current and ongoing research, however any personally identifying information will be removed.
For returns related to Life Extension brand products purchased on the site
Life Extension brand products purchased on the site may be refunded for the full purchase price less shipping and handling if returned with an RMA within 30 days of purchase. Any returns must have the original manufacturer’s seal intact and be in re-sellable condition. Please contact us prior to returning any shipment to receive an RMA. Any returns received without an RMA and that are not in a sealed and re-sellable condition will not be refunded.
